Abstract

Chronic kidney disease (CKD) is a major public health problem and can result in end-stage renal disease with need for dialysis or transplantation. In Europe up to 12% of the adult population had some renal impairment, while in the United States the end stage of CKD has increased dramatically from 209.000 in 1991 to 472.000 in 2004. Diabetes and hypertension are major causes of kidney pathology. Infection, particularly ascending infection, is more common with increasing age, as both immune function declines and associated pathology predisposing to infection, such as obstructive uropathy, becomes more common. Most pathological changes in the kidney appear to be initiated by oxidative stress, followed by an inflammatory reaction. Oxidative stress results from an imbalance between free radicals and their detoxification by endogenous and exogenous scavengers, including polyunsaturated fatty acids (PUFA) Omega 3 fatty acids EPA and DHA . Recent studies showed that omega3 essential fatty acid EPA and DHA supplementation slowed the rate of loss of renal function in patients with IgA nephropathy. Then, studies of omega-3 supplementation in dialysis patients describe salutary effects on triglyceride levels and dialysis access patency. We examined the relationship between total plasma PUFA levels and change in creatinine clearance over a three-year follow-up in the older persons enrolled in the InCHIANTI study, a population-based epidemiology study conducted in Tuscany, Italy. This study showed that older adults with low total plasma PUFA omega 3 essential fatty acid levels have a greater decline in creatinine clearance over three years of follow-up. These findings suggest that a higher dietary intake of omega 3 essential fatty acids EPA and DHA may be protective against progression to chronic kidney disease.

Daily omega-3 supplements may reduce the occurrence of the symptoms of depression in elderly women, says a new study from Italy that adds to the ongoing debate over omega-3 and mood.

According to findings published in The Journal of Nutrition, Health & Aging, depressed women who received daily supplements containing 2.5 grams of omega-3 experienced significant reductions in their symptoms.

In addition, researchers from the University of Pavia also report that omega-3 supplements providing a daily EPA (eicosapentaenoic acid) dose of 1.67 grams and a daily DHA (docosahexaenoic acid) dose of 0.83 grams reported improvements in the ‘quality of life’.

“This [quality of life] observation has never been achieved before and it appears of great value from the clinical point of view, due to the importance of these aspects in the elderly population,” wrote the researchers.

“The concept of quality of life is defined as a perceived global satisfaction and satisfaction within a number of key domains, with special emphasis on well-being.

“Therefore, the amelioration of quality of life in depressed elderly patients after supplementation with omega-3 long-chain polyunsaturated fatty acids is an important .finding,” they added.a joint Anglo-Iranian study reported that depression ratings were cut by 50 per cent following daily one gram supplements of EPA, an effect similar to that obtained by the antidepressant drug fluoxetine, according to findings published in the Australian and New Zealand Journal of Psychiatry.

Recent studies have shown that 1000mg EPA  omega 3 fish oil is as effective as Prozac in the treatment of medium to severe depression . When used together with the anti depressant it was even more effective than either treatment separately .

GPR120 Is an Omega-3 Fatty Acid Receptor Mediating Potent Anti-inflammatory and Insulin-Sensitizing Effects”

Omega-3s may reduce inflammation by acting on a receptor found in fat tissue and on inflammatory immune cells called macrophages, according to research.

The new research published in the journal Cell, suggests the mechanisms behind omega-3’s actions as an anti-inflammatory are due to its action on G-protein-coupled receptor 120 (GPR120) working as an omega-3  FA receptor/sensor.

“Omega-3s are very potent activators of GPR120 on macrophages – more potent than any other anti-inflammatory we’ve ever seen,” said lead researcher Dr Jerrold Olefsky of the University of California, San Diego.

Anti-inflammatory

Omega-3 fatty acids have been long associated with anti-inflammatory effects; however the mechanisms behind such effects have been poorly understood.

GPR120 is a G protein-coupled receptor (GPCRs) – part of a group involved in many important cell functions, and is the target of many drugs.

Previous research has suggested that five GPCRs – including GPR120 included – respond well respond to free fatty acids.

Since chronic tissue inflammation is linked to insulin resistance in obesity, the researchers used GPR120 knock-out mice to investigate if omega-3 leads to GPR120-mediated anti-inflammatory and insulin sensitizing effects in vivo.

Robust effect

Researchers found that GPR120 functions as an omega-3 receptor in pro-inflammatory macrophages and mature adipocytes.

When knock-out mice were fed a high-fat diet and treated with omega-3 fatty acids, they showed all the signs of inflammation and the insulin resistance that leads to diabetes with omega-3 having no effect.

Normal mice on a high-fat diet still gained weight, however, omega-3s “had a really robust effect in preventing inflammation,” Olefsky said.

The study also observed that by signalling through GPR120, omega-3 fatty acids mediate potent anti-inflammatory effects to inhibit certain key inflammatory signaling pathways.

The study reports that omega-3 treatment was as effective – or in some cases more effective – than the popular insulin-sensitizing drug Rosiglitazone.

The researchers noted that activation of GPR120 by omega-3s blocks not one, but all inflammatory pathways.

Interpretation

Olefsky said his team focused on GPR120 from the beginning because of where it is found – in fat tissue and on macrophages. Olefsky noted that if your goal is to fight inflammation then “that’s just where you’d want them to be expressed.”

How these findings can be interpreted for humans is not yet clear, but with a growing trend in omega-3 supplementation and increased dietary intakes of omega-3 a goal for many consumers.

Olefski says it is too early to make any formal reccomendations at the moment, but highlights that he does not see any problem with people taking omega-3 supplementations “as long as it isn’t in enormous doses.”

Olefski said that further research needs to be conducted into several – currently unknown – omega-3 mechanisms. For one, omega-3s seems to block the migration of macrophage cells into tissues – “It’s a remarkable effect, and we don’t know its action,” he added.
